(ii) Proof of Date of Birth [Persons born on or after 1st of October 2023 (attach the following)]:
- Only Birth Certificate issued by the Registrar of Births and Deaths or the Municipal Corporation or any other authority.
4. Proof of Non-ECR Category:
- Matriculation or Higher Educational Pass Certificate
- Birth Certificate (All persons above the age of 50 years)
- Income tax assessment order for last one year (Income-Tax payers)
5. Additional Mandatory Documents:
- If the applicant applying for Re-issue of passport, then the old passport to be submitted at the time of online application and Front and last page of the old passport to be submitted at the time of PSK/POPSK/RPO verification.
- The applicant needs to provide additional proof of documents like Marriage Certificate, Divorce Decree, Notarized Affidavit in case of name change while re-issuing the passport.
- If your passport is lost, stolen or damaged then you will need to provide the original police report and an affidavit that describes the circumstances surrounding the theft, damage or loss of your previous passport.

1. Proof of Address:
- Original Aadhaar Card / E-Aadhaar with valid digital signature (Green Tick Mark)
- Bank Account Statement / Bank Passbook
- Parent’s passport copy, in case of minors (First and last page)
2. Proof of Date of Birth:
- Minor’s Birth Certificate
- A declaration from Head of the Orphanage/Child Care Home on their official letter head (in case of orphan)
3. Additional Mandatory Documents:
- A declaration affirming the particulars furnished in the application about the minor as per Annexure D

✔ The applicant needs to carry original and self-attested copies of parent’s passport to Passport Seva Kendra (PSK), in case parents possess passport.
✔ For minor applicants, documents can be attested by parents.

The applicant should carry the below mentioned documents for offline Passport Application at PSK/POPSK/RPO:
- Photograph: Two recent passport-size photographs are required with pure white background which has to be taken within the last six months and two ears should be visible clearly in photos with 80% face coverage and the size of the photo has to be 3.5cm X 4.5cm.
- Proof of Identity (POI): Aadhaar Card / E-Aadhaar, PAN Card, Voter Id Card (any one)
- Proof of Address (POA): Original Aadhaar Card / E-Aadhaar, Voter ID Card, Water Bill, Electricity Bill, Telephone Bill (Landline or Mobile Post-Paid Bill), Rental Agreement, Bank Account Statement / Bank Passbook, Gas Connection Bill, Income Tax Assessment Order. (any one)
- Proof of Date of Birth (DOB): Birth Certificate, PAN Card, Election Photo Identity Card (EPIC), Driving License, Life Insurance Policy Bond, Matriculation Certificate (Persons born before the 1st of October 2023) and only Birth Certificate (if Persons born on or after 1st of October 2023)
